Wrinkles and creases around the lips and eyes, as well as volume loss in the face are some of the most common signs of aging. Although their appearance is a natural aspect of getting older, they’re not a welcome sight for the cosmetically-inclined and the youthful-at-heart. Facial contouring is achieved primarily through the use of dermal fillers. Dermal fillers come in a variety of different formulas and compositions, each of which is designed to correct a certain concern or to treat a specific area of the face. Fillers essentially act as “synthetic fat” that fill out areas affected by volume loss or enhance areas that have always been naturally deficient. Dermal fillers offer a non-invasive way of contouring, or reshaping your face, to enhance your best attributes.

What are injectable fillers?

Injectable fillers are safe, FDA approved, minimally-invasive treatments that can help contour your appearance to enhance your existing, beautiful features. Injectable fillers are gel-like substances that are most often used to rejuvenate an aging face. As you get older, your face loses a significant amount of fat, and collagen and elastin production slow down. Collagen and elastin are vital proteins found in the skin that form a framework to support cells and tissue. With the loss of these proteins and fat, the youthful, round appearance of your face begins to fade away over time.

What can injectable fillers be used for?

Cosmetic injectable fillers are mainly used for the following:

Softening facial lines and creases
Lifting cheeks and/or volumizing temples
Plumping lips and/or defining their borders
Filling out sunken scars
Decreasing shadows under the lower eyelids
Contouring the nose
Making the face appear more symmetrical
Restoring volume to the back of the hands
Restoring volume to the ear lobes
Defining the jawline
Rejuvenating the vulva to add volume

What injectable fillers do you offer?

Depending on your goals, Dr. Khorasani offers several different types of dermal fillers. Each filler is unique because of its ingredients, texture and intended treatment area.

Juvéderm

Juvéderm is among the most popular injectable fillers. Juvéderm products are formulated with hyaluronic acid, which is a substance the body naturally produces to supply elasticity to the skin. As the body ages, it produces less hyaluronic acid, which results in a loss of firmness. Juvéderm is used for moderate to severe cases of facial wrinkles. Juvéderm Ultra is Dr. Khorasani’s favorite filler for lip enhancement while Juvéderm Voluma is his favorite filler for cheek enhancement. Depending on the type of Juvéderm used, results may last for approximately nine months up to two years

Belotero

Belotero is a hyaluronic acid filler used to temporarily smooth lines around the lips, chin, neck and around the eyes. While Belotero is similar to Juvéderm in some ways, it possesses a lower density, which makes it an excellent option for addressing fine, superficial lines and wrinkles while maintaining normal movement and facial expressions. Since Belotero is crosslinked, or manufactured, in a different fashion than other fillers, it may be used more superficially without causing the Tyndall Effect (bluish color of many fillers when injected superficially). Belotero is Dr. Khorasani’s top choice for treatment of fine lines on the neck. This injectable filler lasts an average of six to twelve months.

Restylane

Restylane is another popular injectable filler that uses hyaluronic acid to add plumpness to the skin. The Restylane family of fillers contain five products designed to treat different concerns including: restoring volume to the cheeks, minimizing nasolabial folds, enhancing the lips, correcting hollowing below the eyes and smoothing lines around the mouth. Restylane is Dr. Khorasani’s first choice when treating the orbital area. It does not absorb as much water as many other fillers, therefore, post-injection swelling around the eyes is much less compared to other fillers. Restylane Defyne is a newer, more cohesive gel in the Restylane family and is often an excellent choice for areas with constant movement, such as the nasolabial folds. Restylane Lyft is a higher density product best suited for nasal augmentation. Most of these products last for approximately six to eighteen months, depending on the volume used and the area treated.

Bellafill

Bellafill is composed of bovine collagen and polymethylmethacrylate (PMMA) microspheres. It also contains the local anesthetic lidocaine. While the body naturally absorbs other injectables over a relatively short period, Bellafill is long-lasting and can give the skin an enhanced appearance for up to five years. Bellafill® is considered a semi-permanent filler and has five years of post-FDA approval safety data published as well. Bellafill is often used for volumization of the nasolabial folds and temples, but has also been approved to improve moderate to severe acne scars. However, unlike other fillers, you’ll need a skin test to ensure that you're not allergic to the collagen component. Bellafill is Dr. Khorasani’s favorite filler for the treatment of depressed scars.

Radiesse

Radiesse is an injectable filler that contains microspheres made up of phosphate and calcium. These two substances are naturally found in the body and help stimulate your skin’s collagen production. Radiesse immediately fills out areas that lack volume, and as time goes on, the microspheres encourage the body to generate new collagen. Depending on the person, Radiesse can last for one to two years. It’s a great treatment for fat loss in various parts of the face like the temples, jawlines and for lifting the jowls by treating the lateral cheek bone. Radiesse is Dr. Khorasani’s filler of choice for general lifting purposes.

Sculptra

Sculptra contains a biocompatible synthetic substance that’s used for stimulating collagen production. This same substance has been used in absorbable sutures for the last 50 years and has an excellent safety record. Unlike other fillers, Sculptra does not initially offer significant volumization. It works by stimulating your collagen and therefore, the results are gradual, but significant. Patients are typically treated over three separate visits, six weeks apart. The final results are evident approximately six months after the last treatment. Results can last up to two years; however, most patients do not go back to their original baseline and some residual volume tends to last for years. Sculptra is Dr. Khorasani’s filler of choice for global volumization of the face and buttocks.

What can I expect from an injectable filler treatment?

Because cosmetic fillers are minimally-invasive, side effects are generally mild, including:

Bruising, which can last from five to fourteen days (check out our tips on how to prevent bruising from injectable fillers)
Swelling, which is region-dependent and is most significant when treating the lips (usually lasts about 72 hours)
Asymmetry, which can be amended in a follow-up visit with enzyme injections that dissolve filler, or a touch-up of additional filler

In a consultation, Dr. Khorasani will conduct a thorough examination of your areas of concern and discuss the improvements you hope to achieve. This will help him determine which type of filler is best suited for your needs, how many syringes you will require and the optimal injection placement. He will then use an ultra-fine needle or cannula to administer the filler. The new formulations of most fillers include a numbing agent, which helps with any discomfort. Topical numbing cream can also be used to enhance your comfort during treatment.

Most patients can return to normal activities right away. The results achieved with injectable fillers depend on the filler material used and the amount administered, as well as the location where it is injected. Some fillers produce immediate results, while others may take some time or require a series of treatments to reach their full benefits. On average, most fillers last six to twelve months, especially those containing hyaluronic acid that will eventually be processed by the body. Synthetic fillers can last somewhat longer.
